- Initially, visit your nearest Benazir Nashonuma Markaz, which is often located in district-level government hospitals.
- Ask for assistance and proceed to the relevant counter to complete your registration.

- CNIC (13-digit ID) of the mother.
- Child’s B-Form.
- Child vaccination certificate.
